Attached are materials you may find helpful if you are filing for FMLA leave. Please forward all paperwork to Human Resources as soon as they have been completed by yourself or your health care provider.
Please be aware that the district’s “Leave Year” for ALL employees is July 1st through June 30th. We no longer operate on a “rolling” calendar.
Also, be aware that you must contribute toward your health care premiums if you are not in pay status.
